How The Grail’s Multi-Cancer Early Detection Lab Test Work

The Galleri test involves sequencing DNA fragments present in the bloodstream. Subsequently, a machine learning algorithm is utilized to determine if these DNA fragments are from cancerous or healthy cells. In the case of healthy cells, the system further predicts the specific kind of organ tissue associated with the cancer signal.

When cells in the body die, whether they are healthy or sick, they release small amounts of DNA into the bloodstream. This is called DNA shedding. If there are cancerous cells in the body, they also release DNA, so both healthy and cancerous cell fragments may be found in the blood. Methylation is involved in determining whether each DNA fragment in the blood came from a healthy cell or a cancerous one.


Methylation is a natural process that involves the addition of methyl groups to the DNA molecule. This process may selectively activate or deactivate certain areas of DNA, so adjusting the function of a DNA segment without changing its sequence. Demethylation often turns genes “on,” while methylation turns them “off”.

An instance of this would be doing screenings on individuals who have several family members with a history of breast cancer, in order to identify any modifications in the BRCA1 and BRCA2 genes. These genes are recognized for increasing the likelihood of developing breast cancer and certain other types of cancer.
  • An individual who has previously been diagnosed with cancer, especially if there are further signs that the disease may have been caused by an inherited mutation (such as a strong family history, early detection, or an uncommon condition, such as male breast cancer). Genetic testing can determine an individual’s predisposition to specific forms of cancer. Assessing the need for mutation testing might also be advantageous for other relatives.
  • Family members of an individual who carries a genetic gene mutation that has been demonstrated to increase their risk of acquiring cancer. Testing can help individuals determine if they need screening tests to detect cancer early or if they should focus on reducing their risk.
